Superficial Thrombophlebitis and Deep Vein Thrombosis (blood clots) are other potential complications from varicose veins.

Varicose veins are prone to developing Superficial Thrombophlebitis (SVT) as well as Deep Vein Thrombosis (DVT). Superficial Thrombophlebitis is characterized by redness of the skin, tenderness, and pain along the affected superficial vein just below the skin surface. As opposed to deep veins, blood clots in the superficial veins are less severe and generally do not break off and go to the lungs.

Deep Vein Thrombosis or DVT on the other hand is a more severe condition. The vein in the leg still becomes inflamed and forms a blood clot. However, Deep Vein Thrombosis has the potential to cause a “pulmonary embolism” – a potentially life-threatening condition – if the clot breaks loose and travels to the right side of the heart and eventually migrates to the lungs.

A landmark study was published in JAMA, a very reputable medical journal, that demonstrated how people who have varicose veins have five times (5X) higher risk for clots in the deep veins that can be potentially fatal. What is Chronic Venous Insufficiency (CVI)?

Chronic Venous Insufficiency (CVI) is a medical condition in which the veins in the body are unable to pump oxygen-depleted blood back to the heart, often resulting in chronic swelling in the legs.

How is CVI related to Varicose Veins?

This is followed by discoloration around the ankles and feet. Left untreated, this may result in stasis dermatitis (dry, flaky rash), phlebitis (blood clots), and ulcers (skin sores).

Due to increased pressure of blood in the legs, even a minor injury may result in profuse bleeding. Poor circulation may cause frequent skin infections and sores that do not heal.

Leg, calf, foot, or ankle swelling (Edema) is a common sign of varicose veins.

The normal flow of blood in leg veins is against the force of gravity; from the foot to the thigh and then up towards the heart. When veins are stretched and enlarged (become varicose), they are too weak to overcome the force of gravity causing the blood to flow backward and pool in the legs. As the pooling continues throughout the day, the pressure inside the vein walls continues to increase. Eventually, some blood and other fluids can leak out of the vein into the surrounding tissue.

edema swellingThe pooling of blood, increased pressure and subsequent “leakage” causes swelling in the ankles and feet as gravity pulls the fluid down. As leakage continues, swelling can also be noted in the lower calf region and extend to the knee or even the thighs.

Swelling is more noticeable at the end of the day, especially if you have spent much of it sitting or standing still. When you elevate your legs, for example when you are sleeping, gravity is no longer a force working against the flow of the blood in your leg veins hence swelling is reduced. However, when you sit or stand, the swelling recurs. Other factors that can contribute to increased swelling include lifting or straining, pregnancy, heat (such as hot baths or sauna), and hormonal changes (that further relax and stretch the weak veins).

phlebectaticaDuring the initial stages of edema, graduated compression stockings may be enough to diminish the tendency of the veins to stretch and control edema. Over time, patients may notice increasing leg fatigue, swelling, thinning of the skin, and discoloration of the overlying skin. Complications such as painful blood clots in the vein, bleeding from veins close to the skin surface, and nonhealing leg sores may develop. Many patients may also notice unsightly spider and varicose veins (Corona Phlebectatica).

Because of the potentially serious and long-term complications of varicose veins, proper treatment is important, and the best results are obtained before complications have already developed. A Vein Specialist can use a Duplex Ultrasound to check “leaky” varicose veins and treatment options.

Left untreated, symptoms of varicose veins such as pain, fatigue, and swelling of the legs or ankles worsen over time. You may develop venous ulcers. Ulcers due to varicose veins are painful and difficult to heal unless the underlying venous backflow is corrected. Blood that pools in the varicose veins is depleted of oxygen and nutrients. The walls of these veins are already weakened and increased pressure allows red blood cells and fluid to leak into the tissues of the leg. This in turn causes swelling, pain, tingling, and numbness in the legs and feet. 

Chronic inflammation that develops from leakage makes the skin dark and discolored, commonly known as hyperpigmentation. Over time, the skin and surrounding fat under the skin can also get increasingly irritated and inflamed turning the tissue rubbery and firm; a condition called lipodermatosclerosis.

Another possible consequence of untreated varicose veins is spontaneous bleeding from the varicose veins. Bleeding may occur even from a minor injury to your legs and cause significant loss of blood in a short amount of time.

Alsara Vein Clinic believes in the comprehensive treatment of venous disorders. Our goal is to offer long-lasting results. We do not believe in band-aid treatments. Prior to performing microphlebectomy, we will examine the larger deeper veins in the legs using ultrasound. If there are problems with the deeper (feeder) veins, those must be addressed first, otherwise, the chances of the veins returning or new varicose veins developing in the area are significantly higher. The deeper saphenous veins are typically sealed using a laser (endovenous laser ablation or EVLA). When microphlebectomy is coupled with EVLA, both procedures may be performed on the same day or scheduled a few weeks apart. A study using 76 patients demonstrated that 58.2% of legs treated with EVLA alone did not require further treatment with microphlebectomy. In most cases, EVLA allowed the varicosities to decrease in size or disappear altogether.

Microphlebectomy is a permanent fix for the varicose vein. The vein cannot come back. Other veins may develop and even appear in the same location, but those that have been removed are gone for good. If new veins should develop, they can easily be eliminated with sclerotherapy injections or microphlebectomy.

The procedure itself typically takes less than one hour to perform and requires no downtime. Further, microphlebectomy may be covered by medical insurance if you are experiencing symptoms such as aching, burning, itching, or swelling in your legs caused by varicose veins. There may also be some incidence of blood clots or bleeding. If symptoms are not present, the procedure will be considered cosmetic hence insurance coverage would be unavailable.

Varicose veins are a common cause of secondary RLS (restless leg syndrome). For those who have varicose veins, the symptoms of restlessness in legs center on the fact that leg muscles and tissue are not getting enough oxygen- and nutrient-rich blood. That "pins-and-needles" feeling is their body's way of telling them they need more blood flowing through their legs, and it creates an urge to move, making it difficult to sleep at night. In addition to cramps ("Charley horse") and restlessness in the legs, varicose vein sufferers may also notice heaviness, excessive tiredness, and aching in the legs. Left untreated the underlying venous insufficiency (pooling of blood in legs) may cause swelling, clots, and sores.

Primary Restless Leg Syndrome (RLS)

Some patients have Primary RLS (restless leg syndrome). This means that the cause is unknown. In addition to varicose veins, there are other secondary causes of RLS including iron deficiency anemia, kidney failure, side effects of certain medications (such as medications used to control depression), peripheral neuropathy, and dehydration (inadequate fluid intake or caffeine intake).

Treatment of varicose veins may alleviate the symptoms of RLS (restless leg syndrome). One study found that 98% with RLS (restless leg syndrome) and varicose veins noted improvement after treatment of varicose veins with sclerotherapy.* Newer treatments such as endovenous laser ablation are more effective than sclerotherapy and offer longer-term solutions. The use of medications to mask symptoms of RLS (restless leg syndrome) may cause side effects without addressing the underlying cause. Other ways to manage symptoms of restlessness in the legs include calf exercises, weight loss, and the use of graduated compression stockings.

Restless Legs and Leg Cramps are Common Symptoms of Varicose Veins

crampsLeg cramps and restless legs syndrome (RLS) are usually the first symptoms of varicose veins. One study showed that people suffering from varicose veins have a sixty-seven percent higher chance of experiencing cramps than those without. The same study reported that an overwhelming ninety-one percent experienced cramps most frequently in the calf region.*

Healthy veins in the legs collect de-oxygenated blood and return it to the heart and lungs where it is replenished with oxygen and nutrients. Varicose veins are “leaky pipes” in our legs that allow blood to pool and become stagnant. Due to the continued buildup of pressure in the deoxygenated blood and toxins, muscles in our legs begin functioning improperly. This results in the muscle cramps and restlessness many of our patients have experienced in their legs.

Since pressure is highest after prolonged sitting or standing due to the force of gravity, leg cramps and restless legs are noticed by patients more often at the end of the day. There can be other causes of cramps and restlessness in the legs.

However, if varicose veins are the contributing factor, the use of medications such as Requip, Mirapex, or Quinine may simply mask symptoms of RLS without addressing the underlying cause. Supplements, such as magnesium and calcium, may help relax the muscles and lead us to believe that nutritional supplements alone will rectify the problem. Unfortunately, the poor blood flow devoid of adequate oxygen remains uncorrected and the underlying disease may evolve into more serious problems such as blood clots, bleeding, infection of the skin in legs, and sores that are slow to heal.

Treatment of varicose veins may alleviate the symptoms of restless legs (RLS). Healthy veins then take over to correct blood flow back to the heart, resulting in symptom improvement. How Does This Vein Treatment Process Work?

When the injectable agent comes in contact with the inside lining of the vein, the treated vein collapses and is soon replaced by a thin cord of fibrous tissue. The vein is now permanently closed. This process is called sclerosis, hence the name "sclerotherapy". The treated vein is then slowly absorbed by your body and eventually disappears.

When is Varithena® recommended?

Varithena® is not used for all forms of varicose veins. It is approved for the treatment of incompetent great saphenous veins, accessory saphenous veins, and visible varicosities of the great saphenous vein (GSV) system above and below the knee. Typically, Varithena® is used for residual varicosities after the truncal veins have been sealed with other treatment modalities such as thermal ablation (EVLA, RFA) or VenaSeal™. What happens after treatment?

What is Injection Sclerotherapy?

Cryo SclerotherapyInjection sclerotherapy is the quality standard for spider veins treatment. Using a special light, the vein is isolated and a small amount of medication (sclerosant) is injected inside the vein. The medication "closes" the vein and stops the flow of blood.

Although the appearance of treated spider veins improves (diminishes) immediately, the spider veins are slowly absorbed by the body over a course of several weeks or months. Typically, several veins can be treated in one session and more than one session may be required depending on the extent of your spider veins. It is also possible that repeat injections in a single vein may be necessary since some spider veins are more "stubborn" than others.

spider veins treatmentSpider veins are the smaller versions of varicose veins caused by the same risk factors as underlying varicose veins. Based on the size of the surface veins (commonly referred to as “spider veins”), veins may be bluish in color (venulectasia), green (reticular veins) or red/purple (telangiectasia).

How are Spider Veins Treated?

At Alsara Vein Clinic, we have almost every treatment modality for the treatment of spider veins. Over the years, we have realized that “inside-out” is the best approach to give you an optimal, long-lasting outcome. We first scan the affected areas using ultrasound looking for any “feeder” veins beneath the skin surface. If present, we use Ultrasound Guided Sclerotherapy to close an entire cluster of spider veins at its source beneath the skin that is otherwise not visible from the outside. Any remaining surface veins are then treated using Injection Sclerotherapy (the Gold Standard) by injecting medication (Sclerosant) directly into the affected surface veins. We use Ohmic Thermolysis or Surface Laser for any remaining veins that are not eligible for sclerotherapy due to their size or location (near ankle or foot). These modalities use a spectrum of light that targets spider veins from the outside and closes them by heating the hemoglobin within the affected veins.

Occasionally, veins near the surface of the skin are too large to treat with sclerotherapy and not small enough to treat with Ohmic Thermolysis or Surface Laser. Microphlebectomy is a minimally invasive treatment to remove larger surface veins including varicose veins that appear on the skin surface. This is not to be confused with surgical vein stripping, which is a much more invasive procedure performed in the hospital setting under general anesthesia and traditionally was used to remove larger, deeper varicose veins. Microphlebectomy is performed in the comfort of our office. Using very fine instruments, the unwanted bulging veins are removed through minuscule openings without the need for sutures and typically without causing any scarring. The patient is able to resume normal activities quickly and is immediately rid of the unsightly veins that are removed permanently.

On The Day of Your Laser/VenaSeal Treatment Procedure:

vein treatment process medical-shortsWhen you arrive for yourLaser/VenaSeal treatment, you will be asked to change into a pair of comfortable shorts that we provide you. If you choose, you may bring your own loose-fitting shorts.

Next, our vein specialist or our ultrasound technologist may perform a brief ultrasound scan and mark areas of interest on your leg. You are now are ready for our unique multi-step numbing process.

Multi-step Numbing Process

Your comfort is extremely important to us during the vein treatment process. We strive to reduce any discomfort as much as possible. That's why Alsara Vein Clinic has developed an exclusive multi-step numbing process to ensure your comfort every step of the way.

  • 1. Cream

    Starting even before your procedure begins, we apply a cream to the skin on your leg. This cream is produced especially for us, and begins the numbing process while preparing your veins for a smooth procedure.

  • 2. Spray

    Immediately before any needle touches your skin, we utilize a special spray which further numbs the surface of your skin, further removing any discomfort.

  • 3. Technique

    While injecting more numbing medicine into the skin, we use a special technique which numbs the skin area for the next injection. That way, your skin is numbed from outside and in.

  • 4. Lining (Laser treatment only)

    Just before the laser is used, we surround the problem vein with a special medicine which numbs, and provides a protective barrier between your vein and the surrounding tissue. Now, numbing is complete, and the laser energy is safely contained right where it needs to be.

Nitrous Oxide "Laughing Gas" (Optional)

laughing gas availableAlthough the vein treatment process offered at Alsara Vein Clinic generally involves little or no pain, we also offer the use of Nitrous Oxide to help take the edge off a patient’s anxiety, helping him or her to relax through the EVLA procedure.

Nitrous Oxide is the oldest, safest, and most fast-acting sedating agent available. It has been used for centuries and is currently used by over one-third of all dental practices.

Patients experience a sense of well-being, feel more relaxed and somewhat less aware of their surroundings, as well as less responsive to any discomfort.

Nitrous oxide is breathed through a nasal mask and, after a state of relaxation is reached, local anesthesia is administered. Complications with nitrous oxide/oxygen are extremely rare. Its sedative effects are completely gone five minutes after it is stopped. It is eliminated through the normal breathing process. Patients are typically able to drive themselves home or to work. This option is available for an additional charge at your EVLA visit.

Needle-free Numbing System (Optional)

Laser Procedure:

During the laser procedure, you'll get to wear a super cool pair of protective glasses. As a bonus, we’ll give you a lollipop of your choice. This helps counteract any odd taste or smell, which can be a byproduct of the laser during the procedure that some patients may experience.

Venaseal Procedure:

Since the Venaseal procedure doesn’t involve the use of heat, there is no need for eye protection or dealing with that funny taste or smell. But if you’re feeling left out of the lollipop fun, just ask! We’ve got you covered.

As soon as your laser treatment is complete, we will obtain some ultrasound images to ensure the target vein was treated successfully (yes, the effect is that fast) and help you with dressing and stockings. You are now good to go back to work (or shopping!)

Home Instructions

We will go over the easy-to-follow instructions before you leave our office.

You are welcome to resume normal activities immediately, including walking, low-impact activities, standing, or lifting moderate weights. In fact, staying active is crucial in keeping your calf muscles (leg pump) working. Now that we have shut down some of the bad veins in your superficial venous system, the good veins in your deep venous system will take over and rely on the pumping mechanism of your calf muscles more than ever before.

Although you are not quite ready for a marathon or heavyweight championship (or our Hot Leg Competition!), you will likely notice improvement as early as the night of the procedure. So go ahead and enjoy your legs!

In case you forgot to ask us something or have any concerns, there is no need to worry. We will call you the day after your procedure to check and see how you are doing and answer any additional questions.

On The Day of Your Non-laser Procedure:

Ultrasound Guided Sclerotherapy (USGS) or Spider Treatment

We will offer you a pair of comfortable shorts or you can bring your own loose-fitting shorts. Ultrasound and/or Vein Lite is typically used to identify and mark the areas needing treatment. One or more numbing tools may be used to reduce all possible discomfort. These procedures require less time and preparation than Laser/VenaSeal treatment. Our vein specialist will treat you and determine a follow-up plan. All instructions are provided before you leave the office.

You will receive a follow-up phone call the day after the procedure to check on you and answer any additional questions.

What is Endovenous Laser Ablation Vein Treatment?

Varicose veins treatment using Endovenous Laser Ablation, or EVLA, is a non-surgical and minimally invasive varicose veins treatment that offers an effective alternative to the surgical stripping of veins. It is performed in the office under local anesthetic using our unique numbing system. We also offer needle-free numbing for the best comfort. Endovenous Laser Ablation is the safest, most effective, and long-term solution that science and modern medicine offers for varicose vein treatments.

How Does the EVLA Procedure Work?

Under ultrasound guidance, a thin fiber-optic probe is inserted through a tiny entry point, usually near the knee. Laser energy is delivered to seal the faulty vein so that blood cannot flow through it. This eliminates unsightly bulging and enables the blood to be diverted to healthy veins in the leg, thereby alleviating any symptoms. Immediately after the procedure patients can resume daily activity. Most patients are able to return to work immediately.

How are Ropey (visible) Varicose Veins Treated?

Varicose Veins Poster onlyTreatment of ropey varicose veins may require one or more combinations of the following: Endovenous Laser Ablation, VenaSeal Closure System, Injection Sclerotherapy (including Ultrasound Guided Sclerotherapy and Catheter Assisted Sclerotherapy), and Microphlebectomy.

Large bulging veins that appear on the skin surface as ropey veins are frequently a major branch of a deeper (inside the leg) saphenous vein, such as the Great Saphenous Vein, which is often, but not always, involved. Ultrasound testing is frequently necessary to determine the level of involvement. If the saphenous vein is involved, it would likely need to be sealed first using Endovenous Laser Ablation (EVLA) or VenaSeal Closure System. This step in itself is sometimes enough to “shrink” the bulging vein. Any remaining unsightly veins may be either closed with sclerotherapy (using medication to seal the vein) or sections of the bulging veins may be removed by microphlebectomy. Unlike vein stripping, microphlebectomy is a simple, out-patient procedure performed under local anesthesia. Using very fine instruments, sections of unwanted veins are carefully removed through tiny openings without the need for sutures and with minimal risk of scarring. Vein treatments may be staged (occur on separate days) or a combination of treatments may be performed on the same day if recommended by your Phlebologist (Vein Specialist).

Now you may be able to get rid of the unsightly ropey varicose veins during your lunch break (and start looking for that pair of shorts you thought you could never wear!).

Vein Diseases - Overview

An estimated 60% of women and men suffer from vein disease.

Some of the signs and symptoms include leg pain, achy legs, burning in legs, numbness in legs, leg cramps, heaviness in legs, itching legs, restless legs, tired legs, swelling in ankles or leg calves. Left untreated, it may result in skin discoloration, bleeding veins, sores that heal poorly, and blood clots.


To understand vein disease, you must first understand the function of veins. Arteries are blood vessels responsible for delivering blood to all areas of the body, including the legs. Veins are responsible for collecting the same blood and bringing it back to the heart. Leg veins, located farthest from the heart, have to work extra hard to push blood upward against gravity toward the heart.

Studies have shown that a vast majority of people have genetically weak veins. Now, if you add more risk factors such as standing or sitting for prolonged periods of time, pregnancy, excess weight, and trauma, veins can start to fail. As veins begin to fail, blood starts to pool in the legs resulting in veins that are "varicose"—abnormally swollen or knotted.

Varicose Veins

Varicose veins are large, bulging veins that are often visible on the skin surface, but the majority are weak or damaged blood vessels below the surface of the skin that cause poor circulation and are only visible with help of an ultrasound.

Spider Veins

Spider Veins are smaller versions of varicose veins that usually appear on the skin's surface as small, red to purple blood vessels that resemble a spider's web. While spider veins rarely pose a medical problem, they often signal problems in the larger veins below the skin's surface.

Chronic Venous Insufficiency

Chronic Venous Insufficiency is a medical condition in which the veins are unable to pump oxygen-depleted blood back to the heart, often resulting in chronic swelling of the legs.

Blood Clots

Deep Vein Thrombosis (DVT) is the formation of a blood clot within a deep vein and can be a complication of varicose veins and chronic venous insufficiency. Classic symptoms of deep vein thrombosis include swelling, leg pain, and skin redness of the affected area.

Restless Leg Syndrome

Varicose Veins and Restless Leg Syndrome (RLS) have many symptoms in common: an urge to move your legs, that creepy-crawly feeling, itching, tingling, and restlessness. These symptoms may be an indication of poor circulation, even though you have no visible varicose veins.

Edema (swelling)

Leg swelling (edema) is a common sign of varicose veins. The pooling of blood, increased pressure and subsequent “leakage” causes swelling in the ankles and feet as gravity pulls the fluid down.

Infection / Leg Ulcers

Over time, you may develop leg ulcers. Ulcers due to varicose veins are painful and difficult to heal unless the underlying venous backflow is corrected.
